While working with bowties and barrier management in general over the past decade, some problems keep resurfacing with most clients we work with:

1) Difficulty figuring out who is going to use the risk assessment once it's done to achieve a goal.

2) Keeping risk information from spreading across multiple registers.

3) Risk assessments become static documents and lose their relevance quickly.

4) Maintaining a consistent approach to risk management across a larger organisation.

We've been working hard on a solution for these issues, and we'd like to show what we came up with.
