David is a leading barrier based risk management analyst and Certified Health & Safety Consultant based in Duncan, BC, Canada. He utilizes the BowTie Method to help clients analyze risk exposure, identify and implement additional controls when there is a possibility for barrier failure.

Tom's 50+ years in aviation span government and the private sector: the U.S. Federal Aviation Administration, air traffic control, flight operations, accident investigation, emergency management, safety, and risk management. He retired from the FAA having served as Manager of the Air Traffic Terminal Procedures Branch and the Air Traffic Investigations Division, and provided investigatory support to the U.S. Secret Service, FBI, and Senate and House aviation subcommittees.
He is lead instructor for the EUROCONTROL Just Culture Prosecutors Expert course in Brussels and holds licenses as a U.S. Airline Transport Pilot, Flight Instructor, Aircraft Dispatcher, and Air Traffic Controller. He is the author of the forthcoming book, Disasters Are Built.
